    I am facing a trouble in Index match with date value,

    You need to explain what you want. A non-working formula tells us nothing.

    I need a suitable formula in column K, specified Date in Column I,
    data pull from Range A2 to D133.

    Maybe try at K2

    =IFERROR(INDEX(CuID,SMALL(IF($B$2:$B$133=I2,ROW($B$2:$B$133)-ROW($B$1)),COUNTIFS(I$2:I2,I2))),"")

    confirm by Ctrl+Shift+Enter

    Please confirm that you are still using Excel 2007.

    If interested, here is a non-array version that works with Excel 2007:

    =IFERROR(INDEX($D:$D,SMALL(INDEX(($B$2:$B$133<>I2)*10^10+ROW($B$2:$B$133),0),COUNTIFS(I$2:I2,I2))),"")
