Finding values from a table that correspond to an individual's characteristics

Hello everyone,

Appreciate you stopping by and taking a look at my issue. I'm an Excel novice.

Attached is a working example of what I have and what I am trying to accomplish.

You will notice that the second sheet (titled 'Table') has three values (L, M, and S) that correspond to a person's specific $ex (0 or 1) and their age in months. I am trying to find a way to quickly output the L, M, and S for each individual in my data set. The L, M, and S values in gray (columns C-E) on the main sheet are examples of correct values. I am hoping to have those corresponding values appear in columns F-H using a formula.

Any help would be greatly appreciated!
