SUM formula and SUBTOTAL gives long results instead of ##.##

When uploading this spreadsheet to a form, the total isn't consistent in showing only dollar amounts. Is there a way to fix this?

I'm expecting to receive a consistent result of a number formatted like 11.11 instead of it showing as 11.000000001 for example

I don't see 11.000000001 in your workbook. And if there was I should format the cells in a consistent way.

Have you heard of or looked into floating point arithmetic and how it inadvertently but predictably adds digits to what we think should a 3-5 digit number? These questions seem to often come down to floating point artifacts. Solutions usually start with rounding functions.
