Using a DATA dump to create an invoice

Hi There,
just need some help on a fairly complex formula.
I have attached an example - basically i need to create an invoice from the data that has been dumped on the "Data Dump" spread sheet.
I need to use cell C22 on "invoice" to then look at the "Data Dump" to then bring through all dates and data (highlighted in yellow) that belong to that particular member code (C22),
I have been playing with a couple of formula but just can’t seem to get what i need, so any help would be much appreciated.

Is there a reason for not using a mailmerge, or one of the many-to-one Word addins that are designed for this kind of thing?
