Edit EMBED objects

I created this form attached over 3 years ago - however I have forgotten how to edit the EMBED boxes where I can 'click' the boxes. Could someone refreash my mind?

Looks like the .xls file did not attach. Anyway - the issue at hand is that I have an EMBED object that I created that allows users to check the box. Now it just comes up and allows users to edit the EMBED object.
