How does this file format change?

I have an excel file on a shared drive.
The icon looks like an excel icon because it was saved in excel. Recently the icon changed to something else but if opened it still opens in excel. How does this happen. I did a save as so I have the excel icon again.

Keep in mind I have 5 or 6 other people opening this file throughout the country entering information and then saving it.

I will attach a screen shot of the two files. Can someone tell me how this is happening. Is someone in another location saving it in a wrong format?

The icon only shows what Windows "believes" about the extension of the file
you are looking at. It's called "File Associations". I can associate PDF
files to Microsoft Word files, if I want, but that does not mean it WILL
open them, it only means I told Windows to use Word to open them. If you
re-associate XLS file extensions to Excel, all should be okay. See
    "Associate the File Types":

If everyone sees some other icon, then likely the file extension has been
    changed.

    Hope it helps!
