Is there a way to make a professional document?

I want to find out if there is a way to make a document that has locked text on it with text boxes and drop-down boxes that looks like a professional sheet and not an excel spreadsheet.
It would be used for dietitians who would fill it out for patients they see on a day-to-day basis. So, in other words the form would have something like:

Name: (then a text box for the name)
Date: (a pull down box with selections of the date)
Diagnosis: (text box to fill in symptoms or something)
    ....etc.....
(p.s. the "Name: and "Date:" and such would be locked so no one could change that)
I am not sure if this can be done in excel and be made to look like a more professional sheet and not so much just a bland excel spreadsheet. Or, if it can be done at all in excel...

    Thanks,
    Ryan

It sounds like you want to create a UserForm. If you are currently using a spreadsheet to store the data, posting it (removing sensitive information) would be the best way to give folks details about what you want it to do.

The help system for the VB Editor has instructions that can get you started. keyword :=UsedForm

It can emphatically be done in Excel - they're called Dictator applications, and they don't look anything like Excel. Creating one requires a thorough knowledge of userforms, and hiding, customizing, and restoring menus. Not for the faint of heart.

If you're ready, I suggest you buy Stephen Bullen's Professional Excel Development.
