Merging Rows with similar information.

Hi

I am trying to streamline an address list so as to only have one row per address. This will make postage more easier to manage. Can anyone help? I have attached a sample list.

Essentially I want to get from this:
First Name Surname Address 1 Address 2
Joe Bloggs 1 Main Street
Jane Bloggs 1 Main Street
Jack Cloggs 2 Main Street
Jim Floggs 3 Main Street
Joanne Floggs 3 Main Street
James Floggs 3 Main Street
Tom Jones 4 Main Street
Tina Smith 4 Main Street
Jeff Kloggs 5 Main Street
Janet Kloggs 5 Main Street
Ted Murphy 5 Main Street

To this:
Joe and Jane Bloggs 1 Main Street
Jack Cloggs 2 Main Street
Jim, Joanne and James Floggs3 Main Street
Tom Jones and Tina Smith 4 Main Street
Jeff, Janet Cloggs and Ted Murphy 5 Main Street
