LOOKUP TABLE help

I have attached a file with two worksheets. I would like to return a value in the first worksheet that considers more than one condition. In the first tab, there is a column where some of the values are combinations (eg. E501/E502). I want to be able to "decide" which side of the back slash is the correct one and return that decision, ie. E501 or E502. In the lookup table, I have included all the possible combinations I will encounter in column A, and I want to return the answer from column B in the look up table. However, this answer from column B will depend on the age groupings from column C. HIghlighted in yellow is a description of the age definitions, in hopes that this will help explain the categories better. Any help is much appreciated!
