Countif formula help needed!

I am attempting to create a countif formula if the data meets one requirment and then any one out of three other requirments. For example, I want it to count if a store was located florida and was either opened,closed, or moved in 2011. I have tried using various countif/countifs/sumifs but cannot find anything that will work if one criteria must me met and then one of the other three needs to be met. Also, I only want it to count 1 time for each successful count, so if the store was in florida and opened in 2011 I want it to count for 1. Any help is greatly appreciated!!
