I need help re arranging this.

Line 1 is item number
Line 2 is warehouse
Line 3 is Qty

I need it in this format

Item # , Qty
0010041, 0000054


But I just wanna pull the Qty value from warehouse 004.

thanks works great but on large scale excel sheets having difficulties. The excel sheet i uploaded is a very small portion. It has over 75000 sku's can you help me with same idea in the excel sheet i have attached. Just looking to pull the sku and qty out of location 4 on 75000 skus.
