Pulling data for a pivot table

I have a worksheet that I have been trying to pull the data highlighted and form a pivot table. One of my problems is that I there are some blank cells in the table. I also have had a problem with going down and getting the information for more than 5 cells per person. Any help would be awesome. Im stumped on this one again. Hope this was fairly clear???

Oh I am trying to get the data for each person. This is just a sample portion the whole work sheet is like 4000 cells. Thanks
