Indirect Cell Reference

Okay, I have a formula which returns the text "D4".
It looks a little something like this:

="D"&MATCH(A1,C1:C5,0)

I want it to find the text in A1 in my list in column C, and give me the corresponding value from column D. Can I get it to display the contents of cell D4, rather than saying "D4"?
Is there a way to go about doing this just in a formula? I don't want to have to use a program, or visual basic or anything like that. Thank you anyone who can help.
