Excel 2007 : Index and Match-prices and catalog numbers

I have a spreadsheet I am going to attach that. Under the Entry form. I have prices and catalog numbers and quantity. Under my Proposal tab. I have the form for somebody to fill out. I need two options to be true in order for the price to come up. So if you buy 500 instead of 250 you get a price break. The problem is it works under one of the matches but not both. My formula is
