    User form with option buttons

    Good evening.
I'm trying to create a form using option buttons (see attached)

As an amateur on how to use option buttons properly , much less organizing them to make them more presentable. I'm also faced with having to add unnecessary columns in order to get certain areas to line up.

I must let you know that i'll be using this form constantly after construction to print out.

Can anybody offer me any advise please into what i'm doing wrong.

    Thanks in advance

Read up on using Named Ranges to create Data Validation lists.

On sheet 2 I've put your 3 groups of items in a list and given each group a "name". (See Insert > Name > Define)

Then I used those lists to create drop down data validation selection boxes on your sheet1.
