match function - multiple lookups

Hi - back again!

I have a worksheet with an INDEX MATCH formula. I'd like it to bring back the results for the years 2009-2011 based on which customer is selected and which material is selected. It works perfectly for the customers but only for the first material.

I know there must be a problem with my formula (cells C18:N20) - but where have I gone wrong?

I enclose an example for reference.
