One way might be to use helper columns.
In both "Action_Plans" & "Issues" insert a column at the begining then in A2
Drag/Fill Down
N.B.
If you expect more than 9 actions for any one issue.
Copy "Action_Plans" Column A and Paste Special > Values to "Summary" Column A.
Sort this column ascending.
In B2
In C2
In D2
Complete your table with similar lookups adjusting the column ref to your requirements
You can hide the helper columns if you wish.
The summary will need to be manually updated and sorted (Column A only) when your data changes, but you did ask for ......
