contact information form (template)

I have a data sheet i use for contact information for all of our business locations. It has column headings for each piece of information i need for each location. I have the form icon set up and use it to enter information for new locations.
Here is my question;
Is it possible to set up a sheet so it would look like the form created when I use the form button? That is the layout I would like to see when I open the sheet without having to use the form button.
