Hi Jsmaage, welcome to the forum.
Why is your lookup table laid out that way? Merged cells? Never use them if you don't have to.
Why are there three values for each combination? 5 & 1 could be 100, 1000 or 10000 -- why is your expected result 10000? 4 & 3 could be 104, 1040 or 10400 -- why is your expected result 10400?

To answer your second question, those three numbers in my original worksheet are wages/dollars. First being hourly wage, second being monthly and third value annual. I'm looking for the annual (third) value in each field to be inserted into the first sheet (WS1).
Richard - That way I know of...but if I ever make changes to the two number columns in WS1 the value displayed will no longer be right. The formula would have it looking in the wrong cell, I was hoping to link them somehow if that makes sense?
Try this formula in F3 and fill down:
=INDEX(WS2!$B$2:$F$10,MATCH(WS1!E3,WS2!$A$2:$A$10,0)+2,MATCH(WS1!D3,WS2!$B$1:$F$1,0))
