Custom Form with Submit Button, Email and then Submit to Database

    Custom Form with Submit Button, Email and then Submit to Database

    I have attached the form.

    I would like to add a submit button to the bottom of this form.

    When the submit button is pressed I would like for it to email the form to a specific person, then I would like for the information that was entered on the form to be stored in a data base.

    I have read everything I can and cannot find anything that will help me to do this.

    Looking for ideas????

    and as an option for yr data you can use. see if it helps you!
