Lookup Criteria in cell

I am trying to create a Biggest Loser tracker. I am trying to add job performance metrics into the sheet as an added incentive.

In my example sheet, on the DATA sheet in cell BJ5, I am trying to add in a formula that looks at Targets 1 and 2 for week 1 (Cells G5 and H5) and determines the respective point value on the Targets sheet.

*Edit* Also, I am trying to avoid hard coding the criteria into the formula incase I need to change it at some point.
