Input box to collect comments in a new worksheet

Is there a simple way to create an input box for a comment regarding the contents of a cell?

For example, I need to capture comments from two different persons regarding their thoughts on the attributes in separate cells. These comments would be deposited in a new worksheet next to the attribute so that one could easily correlate the comment with what they are commenting about. I attach a sample spreadsheet to better illuminate the idea.
