COUNTIF to identify non-repeat values

Hello,

I need a clean COUNTIF formula to identify if a set of values across a row do not occur again in columns if they have a specific condition.

If Column E = "Accepted" and Last Name & First Name do not occur again, then Column F should indicate "Open Case"
