    Icon Set in conditional format

    First time I've used icon set in conditional format. I am using the red, yellow, and green dot option (3 traffic lights unrimmed).

    I want percentages greater than or equal to 45% be green, between 42 and 45 yellow, and below 42, red. I've tried this in several different ways on multiple spreadsheets, and my yellow isn't showing up, and some conditions that should be red are actually green.

    I can attach the spreadsheet, but since it doesn't work for me on a blank workbook, I would appreciate guidance on what I'm doing wrong on the principle of icon set.
