    You are saying a couple of things that contradict each other.

    What do you mean by "image"? Because dumping things into image file formats is like, a super-terrible thing for you to do, because it makes the data basically inaccessible from the computer's point of view.

    Do you want Handwriting Recognition Software?

    Are you building a database?

    Why are you asking about going from paper => excel => HTML form? The flow of data, as described, sounds irrational.

    So you want to take your excel data, and print it in a different format, right?

    This is just cosmetic stuff. My advice is, create a new worksheet that's the "output". Format the output as a sheet to look the way you want, and then just link the fields to the input tab.

    If you post the excel spreadsheet, I can put together a quick example of what I mean.
