    Index Formula with Multiple Matches

    I used to write an index formula that had to match 2 or more criteria, but I can't remember how it worked anymore. It's an awesome formula, so I can't believe I've forgotten. Thanks for any help.

    I've attached a file with names, ID #'s and departement. I'm trying to match the ID to the Department and bring me back a name. I've changed one of the employee names so it looks like she changed departments and got married, but I want to know her exact name for each job. I hope that made sense.

    Thanks for the reply, but it's giving me incorrect results.

    I typed this formula: {=INDEX($A$3:$A$33,MATCH(F12&G12,$B$3:$B$33,$C$3:$C$33))}

    Do not type the curly braces, enter the formula:
    Formula: copy to clipboard

    On the key board CTRL + SHIFT + ENTER not ENTER.

    I understand, Ctrl+Shift+Enter encases the array with the curly brackets, I didn't type them. It's just not pulling the correct name at all and I'd like to repost my spreadsheet with the changes, but it won't let me.
