    using excel for home based symptoms diary

    Respected sir,
    May I introduce myself as a primary care pediatrician from S.India with 28 yrs experience.
    Initially I had registered in this forum, with intention of a building a community base electronic health record. [I was warned, correctly by the experts in the forum, that it has legal issues.] Now I have this opportunity and challange to make parents use excel sheet to maintain symptoms diary on a day to day basis. As this is at patient level, it will not have legal issues.
    Asthma is on the rise through out the world, including children. Below 6 years there is no test to establish asthma in children.Only way out is to maintain a diary of events
    There are mainly 5 symptoms. Nose block, itch and leak. 2. Night cough 3. Activity like crying and laughing induced cough. 4. Day time wheeze and 5. Night time wheeze.
    I have attached a sample sheet.
    We generally take medicine as and when required. SOS for short. But in asthma we have to take drugs daily. We don't have proof to say it is asthma in the form of tests. Hence this diary is very important in establishing the diagnosis.
    Now what I want.
    At the top of the sheet: what parents have to do is type p for present and a for absent. An illness requiring drugs is called an attack. If there are 2 attacks in 30 days, 3 in 6 months and 4 in a year, SOS will not work we have to go for daily medications. This data needs to be summarised.
    At the bottom I have written the summary.
    How to transfer the data in the day to day entry to be transposed to the summary coloumn? In the day to day entry, the row is important. In the summary stage, the columns are importan.
    Is it possible to send message from a cell phone so that data can fix into the excel coloumn. We can code each symptom as 1, 2, 3, 4, 5.There only 2 options for each. A for absent, P for present.Some say it is possible.

    you may be better off formatting the data , so you can then run in a pivot table
