Referencing a table to another sheet based on a specific table value

Hello all

First time in this forum! Would love some assistance. I'm physician starting a small database of my patients.

I have 5 total tables. One is a demographic sheet, which everyone will have only once.

I then separate the patients based on the number of respiratory cultures they've had, only 1 or more than 1. Two tables will be for those with only one culture, the other two for those with two cultures or more.

All variables are in the first row like this (I can attach the table if needed):

One_many / Name / Medical_number / Culture_number / Date_obtained / Culture_#

The "One_many" variable will just be a 0 or a 1 (0 for one, 1 for many). The idea is that if this number is 0, the data gets linked to the two tables for those with only 1 culture. If it is 1, likewise, so those tables start filling automatically. If at one point the individual with only one culture now gets another one, I would change his "one_many" variable to "1" and he will be moved to the appropriate table.

All the other tables start with this exact variables. Note that one patient can have many cultures, but since each culture has its own identification number, I can eventually sort by individual and their specific information.

I know how to paste a special link to another table - but any help with this will be appreciated.

Only added 4 rows of data - it would represent 3 separate individuals, and one of them would have 2 cultures, thus 4 cultures.

I'm thinking, since I'm going to associate an individual to a culture(s) anyway, should just keep the first 3 variables - the "one/many", plus two identifiers, such as the name and record number.

Hi,

I took a look at your spreadsheet. I think its a little too overwhelming.

You have a total of 75 different fields in the workbook spread out in 5 spreadsheets. I think it would be better to limit the information in the sheets and then pull the data using reports. I created a color shaded sample of what I am seen so you can see what I am talking about.

If the data base is too long and the columns expand to BW then a user-form or several user-forms would help enter, retrieve, and report data.

Thanks for the feedback. Yes, I know it's long!

Excel does have a form tool, but do not know how to create a form with Excel. For prior studies, I just imported something made from SurveyMonkey, but with at least 300+ patients, a form linked to a database may be better.

I do have FileMaker Pro in my Mac, but was received as a gift and do not know how to use it much. Guess I can import the excel sheet and make a database out of it.

Is the Medical Number variable (MRN) a unique value?

I do have FileMaker Pro in my Mac, but was received as a gift and do not know how to use it much. Guess I can import the excel sheet and make a database out of it.

If you have a database software program go with it. i have never used it but I saw a demo and it looks very good.

Each patient has its own medical record number, and patient's culture has its own ID number as well. One of my subjects has 15 cultures, so it all falls under one unique ID.
