    Option Button (Form Control) - Different Entries

    Hey!

    I am preparing a Survey where I have used Option Button (Form Control).

    I have 5 Options for each Question and I want the answer to be either 1,2,3,4 or 5 based on the input selected which will be reflected in Column E infront of the appropriate Row Number.

    However, when I place more than 5 Option Button (Form Control), it treats it as the 6th, 7th, 8th Button and so on, where I want it to be 1,2,3,4 & 5 again for the next question. (File Attached)

    Can anyone help me with this problem?

    Place a "Group Box" From Forms controls , around each group.

    Tried that too. When I put the group box, If I select any option from 1 Group, it select the same option to other groups automatically.

    Is there a way around it?
