Advanced filter with 2 criteria

Hello - I have a large dataset that I would like to use an Advanced Filter for but I can't seem to figure out how to do it.

Basically I have 3 criteria: State, Date Range, and a list of PCPs that is much smaller than the entire dataset. The list of PCPs is on a sheet called Lists in Column H and the row header is called PCP_Selections. The state and date range are also on the Lists Sheet: state in Lists!C2 and date range in Lists!D2:D3 (begin and end date).

What I would like to do is to use Advanced Filter on my very large dataset and filter on the state value in Lists!C2, the date range in Lists!D2:D3, and the list of PCPs in Lists!H2:H25 (although H25 can vary and is really variable).

Is it possible to use Advanced Filter this way?
