Conditional Formatting based on two conditions

Really sorry, have searched the forum but cannot find anything that quite answers my question.

I have conditional formatting applied to a list of items based on the value in the adjacent column.

However, I have a separate list of items in difference columns where I want to also apply conditional formatting based on the first column

    How about
    Select A4:D9 & use
    =INDEX($G$4:$G$25,MATCH(A4,$F$4:$F$25,0))="Y"

Thank you sooo much. I could not get my head round that. I knew it should be index and match but could not work out where to put the ="Y"
    Perfect!
