need help with Form Template with Macro

Hi,
got this file sometime back, but can figure how the original owner did the macro, can anyone help?

Basically, want to know how we input under 'template' sheet, and when click on botton below, it creates a new record in another sheet. and everytime I create a new entry in 'template' it will be inserted as a new entry in the next line in the 'database' sheet.
