Organising data using Index and Match FUnction

I am stuck with respect to organising my data in excel using index and match. I tried using this function but am unable to get the results. The format of data is such that vlookup and hlookup are also not working.

I'll be very grateful to you if you could help me get around this problem.

I have attached the excel file with the raw data and the index match function that I'm using.

I look forward to hearing from you and do really appreciate your help.

Try this in column N and adapt it for the rest:

    =LOOKUP(2,1/(($A$2:$A$30892=$L$4)*($B$2:$B$30892=$M$4)),$C$2:$C$30892)
