Change your existing CF formula in E6:E25 to:
=AND(B6 <> "", FLOOR(E6, 1) <= TODAY(), NOT(ISNUMBER(MATCH(B6, H:H, 0))))

1) Don't know.
2) Formula change for E6, then copy down:
=IF(C6="","",DATE(YEAR(C6),MONTH(C6),DAY(C6)+15))
3) Highlight B6:E25 and put this CF formula in for the entire range:
=AND($B6<>"",FLOOR($E6,1)<=TODAY(),NOT(ISNUMBER(MATCH($B6,$H:$H,0))))
