Questionnair - With Weighted Questions

Hi,

Trying to create a form that asks the user a couple of questions and allows them to answer (generally high/med/low) using radio buttons. All the answers have a different score attached to them which in turn allows us to calculate a risk/result.

Once a user has completed all the questions I require a 'Submit' button and based on the users score a different message is displayed.

Is this possible to do in excel? I have so far created a table with questions & answers which are weighted differently.

I have attached the start of the question, starting small..!

Is there a tutorial on creating questionnaires similar to this?
