Using Excel option button to prepare a answering sheet

Friends,
I need to prepare an excel sheet for my son to click on a multiple choice answer sheet. Example: An excel sheet with some 100 question with each question has 4 choices. Each choice is linked to an option button. One can click on 1 out of 4 buttons, which can be saved.
