Split data and place in appropriate cells

Hi - I would like to obtain a solution to the attached spreadsheet.
Using the Data in Column D, populate either columns EF and/or GH dependent on the criteria of FT or PT or combination of both.
The attached sheet has a sample of data in column D with the desired outcome in columns E to H.
Hope someone can assist - thanks in advance to anyone willing to give this a go.
