Create a list based on answers from a questionaire

Greetings,
The objective of my workbook is to have a user answer a series of questions with a simple Yes or No from a drop-down. When they have completed the questionnaire, I would like for them to be able to click a button, running a macro that would review their responses and create a list of action items associated with each question they answered "No". It would also be useful to have a second button to clear the list in case the user wanted to start over or adjust their answers. I would prefer the buttons and generated list to be directly below the questionnaire. In the example provided, Sheet1 shows what it would look like before a user completed the questionnaire. Sheet2 shows what it would look like after a user completes the questionnaire and clicks the button to generate the list.

Add this code to a code module and assign your buttons to the appropriate macros. You'll need to save your workbook as a .xlsm.
