i am getting this error while inserting the record please help me out

i have 4 labels and a 3 textboxes

still i am getting the same problem

in lbl2

label1 showing username,one label showing date in userform,label showing Time
another label showing system name

i am trying to storing this data into Mysql Database

sorry for the password protection for userform

i have attached my new workbook in that userform(login)

I think lb2 is not correct field name.
