Add Row & autofill formulas

Good morning,

I'd like to add a button to my spreadsheet that prompts me to fill in a number of fields from my table and then adds a row at the bottom of the table with the information entered. Could someone point me in the right direction? I've tried searching through the forums but couldn't find anything specific enough.
