On clicking submit need to create pivot & summary as shown in attachment

Hi All,

I am trying to create a pivot table with summary as showing in the attachment. I have data in the details tab, so once I click on submit, it should display result as shown in sheet "Summary". Note: Major problem is with labeling & merging the rows 1,2, how do I label them... please provide macro code for it....
