Lightbulb Create / Persolalise a form with data from a table

I have a list of data with order numbers, contact names, addresses etc. Each line is a different order and I would like to be able to either:

Click on a command button situated on each row to open and populate a separate blank form with data from that row

    Or

Click on a command button which then asks which line of data to use to populate a separate blank form.

Can this be done in Excel?

Any help would be extremely gratefull

Attach a sample workbook. Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

    Remember to desensitize the data.

Click on GO ADVANCED and use the paperclip icon to open the upload window.
