    Populate form textbox from database

    Hi guys, complete newbie here...

    I presently have this formula in an excel sheet which gets the description of an item written in cell B5. For this to work I had to install an excel addin which someone else made. I am just listing this here for the data source and table names.

    What I would like to do is to create a form with two text boxes. In one textbox the user would write the item and the other text box would populate the description based on the item input by user in the first text box. The description is located in the database table references in the above sql query.

    Is there any way I can achieve this please?
