Copy data from multiple columns and paste into single columns

Hi,

Wondering if anyone can help with this please:

I have data in multiple columns - basically nominal codes and monthly budgets for each code - I have attached a file of the data ('BEFORE' sheet) and how I need to display it ('AFTER' sheet).

The highlighted columns in 'BEFORE' is what I need to copy and want the data to be diplayed in 3 columns - SUPERKEY, AMOUNT AND DATE.

Thanks AB33, it worked perfectly! Just had to change the format on column 3 to Text so the dates displayed how I wanted them.
