Replace DTPicker with VBA code in UserForms

Part of this question was answered by Forum Guru member JasLake (who has been very helpful), but I don't know how to contact Forum members individually, so I'll post my question to the Forum.

I'm struggling with replacing the DTPicker function in UserForms. I found and downloaded JasLake's frmCalendarv.102.zip file (see link below) and have installed it in my workbook and it is working wonderfully! I recommend it to everyone who is looking for an elegant Calendar look-up solution (and to replace the now defunct DTPicker).
It is working great in all the sheets within the Workbook, but how can I make this work the same way in UserForms?

I've been experimenting, but this is getting beyond my pay-scale.

Here is JasLake's file that I've used to replace all the DTPicker fields in my workbook. I like to know what edits I need to make in order to have the same functionality within UserForms.

I assume that I can share this within the Forum rules?

    Thank you!

    Hi John

Please Note...this Code is NOT my creation...merely an adaptation.

Post #3 of this Thread has an example of a Non Activex Calendar Control in a User Form.
