Hi, in row 2 put these formulas, then just select A2 to D2, and copy down.

Right click on Sheet1 Tab --> View Code --> Paste the code given below on the code window --> Save your workbook as Macro-Enabled Workbook.

Now on Sheet1, in column E, input an Id to populate the information on other cells from Base Date sheet.
