Userform to display Log data; add, edit data, and navigate between records help

Hi,

I'm very new to VBA and am in the second iteration of attempting to create a userform to control the data entry of a Log.
I adapted the code from an old VBA book after failing from a previous version of the form.

I need to navigate 100+ records, display the selected record in the form, allow the user to edit/add missing information, or allow a new record to be created. We are adding proposal number as our unique identifier (format 2012-11111). This code is exact from a book, but it wasn't working, so I added the worksheet reference and still no luck. There are so many ways to do this it's hard to pull it together in my own context.

This form won't display the data at all. Any help is very much appreciated!!!
