    Smile Data Entery with User Form

    I am hoping to get some help doing the following as i am very new to this part of Excel. Using a user form, I would like to have 3 text boxes (return date, RMA and company name) with a command button to input data into a table. Column A would be Return Date and so on. I would like the date to be input into the next available row at the bottom each time. Unfortunately my company refuses to get me any such program so any help you be greatly appreciated.

    It is close but I am looking for something more like the word document (Picture) I have attached if it is at all possible....
