I need to auto transfer all the underlined data to the Response data sheet to the header line under the correct heading. I tried to assign a button to add data but quickly realized I had know idea what I was doing.

To get you started, here's how you could populate columns A to C of the Response data tab from the Intake & Response Form tab:

This would require a lot of data mapping to complete
i started you off in the attached file
just complete the rest of the mapping using the same format as preview row
