UserForm code not entering data

I have a UserForm that is supposed to enter the data in each text box into the last empty row in the sheet when you click "Add". Then a Message box is supposed to pop up asking if the user would like to enter more data.

Right now all it's doing is entering one bit of text the "ClientName". Everything else is staying blank and the Message Box is not popping up.

It was working in another workbook and I copied it over and changed some of the boxes' names, but everything looks right to me...

Hard to say without seeing the userform. You said you changed some names. Do the names of the text boxes in the code match exactly with the textbox names on the userform?
